Indikation: Männliche Stressharninkontinenz mit einer Restfunktion des äußeren Schließmuskels
Das ATOMS (Adjustable TransObturator Male System) ist ein einstellbares Langzeitimplantat zum Einmalgebrauch. Dieses hydraulische Schließmuskel-Kissen-System ersetzt die Funktion des Harnschließmuskels bei inkontinenten erwachsenen Männern.

Das Silikonkissen von ATOMS übt Druck auf den M. bulbospongiosus und damit auf die Harnröhre aus. Diese Kompression reduziert den unwillkürlichen Urinverlust und erleichtert gleichzeitig die physiologische Kontraktion, die das absichtliche Urinieren bewirkt.
Die Schlinge ist selbstverankernd und die symmetrische Positionierung des Kissens unterhalb der Harnröhre wird durch eine 4-Punkt-Fixierung erreicht. Die integrierten Netzarme werden um den Ramus pubicus inferior bis zur Mitte des Implantats zurückgezogen, um das System zu fixieren.
Der angebrachte Port wird im Skrotum positioniert. Über den Port ist das Füllvolumen des Kissens und damit die Kompression auf den M. bulbospongiosus einstellbar. Das Füllvolumen wird vom Chirurgen mit einer nicht-bohrenden Portnadel eingestellt und hängt von der Anatomie und den Bedürfnissen des Patienten ab.
